Please read the following thoroughly and follow the steps below.

To initiate an appointment request with HolisticalVets LLC for your pet, we request that you follow the steps below. Upon receiving your completed Patient Consent Form and pet’s medical records, our team will review the information to determine how we can best assist you and your pet. If we determine that we can be of assistance, we will reach out to you via email to express our interest in offering our services to your pet. This email will include a link to our appointment scheduler, allowing you to conveniently schedule an appointment and you will have the option to request an individual appointment or a package containing either four (4) or six (6) appointments. 

In the event that we find our services may not bring significant benefits to your pet, we will promptly communicate our findings via email as well. At HolisticalVets LLC, transparency is of utmost importance to us, and we are dedicated to prioritizing your pet’s well-being. Therefore, we uphold our commitment by refraining from accepting payment if we determine that our services may not align with your pet’s specific needs and requirements. Please be aware that completing the Patient Consent Form and authorizing your pet’s medical records be sent to HolisticalVets LLC does not guarantee that HolisticalVets LLC will be able to provide services to you and your pet. 

Please follow the steps below:

Step 1: Please click here or scroll down slightly to access and fill out the Patient Consent Form. Upon completion, please make sure to click the green “Submit” button at the end of the form. After successfully submitting the form, a “Thank you” message will be displayed, and you will also receive an email containing your copy of the Patient Consent Form.

Step 2: Please contact your pet’s primary veterinarian and request that your pet’s records be sent to info@holisticalvets.com.

  • Please be aware that only medical records received from a veterinarian will be accepted, so we request that you please ask your primary veterinarian to send your pet’s medical records to info@holisticalvets.com as soon as you submit your Patient Consent Form to allow time for your medical records request to be processed. If any other veterinarians (e.g. emergency veterinarians, general practitioners, specialists, etc.) have seen your pet in the last five years, we kindly request that you ask them to email your pet’s medical records to us as well. 

Step 3: Upon receipt of both your Patient Consent Form and pet’s medical records, one of the following things will occur in the following 72 business hours:

You will receive an email expressing our desire to establish a veterinary-client-patient relationship with you and your pet. This email will include a convenient link to our online scheduler, enabling you to effortlessly schedule an appointment at your convenience. Shortly after successfully scheduling an appointment, you should receive an automated email from HolisticalVets LLC titled “Appointment Confirmation” (if you do not receive this appointment confirmation email, please email us at info@holisticalvets.com). Please note that the credit card you provided during the appointment scheduling process will be charged the indicated amount on the same day as your scheduled appointment.
