Please be aware that only medical records received from a veterinarian will be accepted, so we request that you please ask your primary veterinarian to send your pet’s medical records to info@holisticalvets.com as soon as you submit your Patient Consent Form to allow time for your medical records request to be processed. You should receive an email within 72 business hours from us once we receive both your Patient Consent Form and pet’s medical records. Thank you!
